    Though I agree with D as the answer, the solution doesn't appear to fully capture the related costs.  For Vendor P, in order to obtain 100,000 switches with a 90% pass rate, you would have to test 111,111 (0.9 x 111,111 = 100,000) switches, not 110,000.  The formula should use the term 100,000 / 0.9 in place of 100,000 x 0.10.  This results in a total cost of $4,166,660 for Vendor P, instead of $4.1 million.

    why is the $35 re-accounted for when determining the cost of failure? isn't it already taken into account with the initial purchase of the switches?

    Hi Jessica,
    The $25 charge is the lost productivity, the additional $35 is the cost for the replacement switch … so the production line has to remove the failed switch and replace it, therefore the product in effect used 2 switches to get one that worked.​
